Guess what? err_gfx_d3d_init. At this point I'm convinced its 350.12 causing this. The only thing I'm wondering at this point is why a hotfix hasn't been released yet. The nature of this crash seems rare, at least on my end.

The two d3d DLL files are HLSL compilers that handle shaders, and so it is reasonable to assume that the ERR_GFX_D3D_INIT crash might be related to shaders. Some has suggested reinstalling directx from within the gta folder (CommonRedist), thats what I did, and so far, it seems to be working for me at least.
